I purchased and used Artic MX-6 thermal paste, both to "bind" the aluminum blocks to the case above, as well as for the chipsets (no use of the included thermal pads).
I also used the thermal paste on the bottom, to bind the case to the aluminum case bottom before screwing it into place, with the idea of allowing that lower piece of aluminum to absorb heat with the rest of the case. (link to Artic MX-6 paste: https://www.amazon.com/dp/B09VDLH5M6)
I then used CPUBURN with the latest pi4 firmware set at 1800Mhz CPU/600Mhz GPU for 3 hours at 100% across all 4 cores.
The results were between 38 C and 41 C.
I immediately over clocked the CPU/GPU in config.txt to 2000Mhz CPU/750Mhz GPU and re-ran the test, at this point the case would have already have been pretty heat soaked, this ran for an additional 1.5 hours.
The results are a high of 44.4 C.
The case throughout this testing was just warm to the touch to the temperature of a hand warmer, and can be safely handled without any concern.
Lastly I read many concerns about the MICRO HDMI cutouts, by luck I had on hand "Twozoh Right Angled Micro HDMI to HDMI Adapter" cables (link at: https://www.amazon.com/gp/product/B09PYXLK23).
Luckily these cables are an ABSOLUTE perfect fit for this case, the connector is long enough to be fully seated when the black plastic "shield" is just touching the aluminum siding and boots up without any issue.
I have included photos of the testing at 1800Mhz/600Mhz as well at 2000Mhz/750Mhz, I also included images to show the pi4b board in the case with the HDMI connector I listed, fully inserted with the board pushed all the way to the side of the case, this shows the excess connector that is still available.
There is then a image of the board in the case with the screw holes lined up, at which point the black plastic "shield" on the connector is just touching the side of the case.
The above I feel is evidence of how well this case handles heat, better than other current passive cooling solutions I have seen on the market to date.
I have a total of 4 pi4 boards, and each one of them now sports the same case.
I feel this testing is "case closed" (excuse the punny joke) for any who are wondering if this case really is as good as what they have heard, especially for the price, and I also took the liberty to provide a decent braided 90 degree micro HDMI to HDMI adapter that solves the issue of the HDMI cutouts.
The corners of the case are rather sharp, but nothing that a little sandpaper wouldn't solve.
This case for what it does, with the simple setup I have provided is rather impressive.

I suppose I could have shaved the plastic from the end of the micro HDMI connector until it worked with the stock opening - a couple of mm would have done it. Nope. I used a dremel to enlarge the opening to fit my cable.

Another thing I appreciated is the precision production - all parts fit perfectly. And all screws are the same, making assembly very easy.
The only down side, like others have said, is that micro HDMI cables don't reach in far enough since the Pi connectors don't sit flush with the inside of the case. I had to trim the molding on my micro HDMI cable by maybe 1 mm, and then it worked just fine.
Another potential downside is that it's a bit pricey. But best in class for sure. Not exactly mass produced, so one would expect it to be a little pricey yet.
The Raspberry Pi fits snugly into this case. I haven't had any issues yet with Wi-Fi or Bluetooth connectivity through the metal. I haven't stress tested the Pi in this case yet, but it has decreased my normal operating temperature by 15-20 degrees Celsius. My Amazon Basics Ethernet cable and Logitech wireless mouse dongle fit into their respective ports nicely. My CanaKit USB-C power cable barely had enough insertion past the case. My Ugreen micro HDMI cable did not fit past the case. To make sure both cables were inserted properly, I carefully cut down the molding on both with a razor blade. Having to modify cables may be a deal breaker for some, but was a small price for me to pay to get to use the Gem Pro.

b) Heat sinks from earlier installations need of course to be removed (didn’t think of it, so mounting became a bit messy, no problem after correct removal of the old sinks)
